Physical Therapist – Home Health (Contract)

Position Overview

We are seeking an experienced Physical Therapist to provide high-quality home health services focused on restoring function, minimizing disability, and maximizing patient independence following illness or injury. This role involves evaluating and treating patients using physician-prescribed physical therapy programs in the home setting.

The Physical Therapist will collaborate closely with the clinical team, contribute to patient care planning, and ensure accurate, timely documentation in alignment with organizational standards and patient care goals.

Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide skilled physical therapy interventions in the home setting
• Perform initial admissions and evaluation visits as assigned
• Document clinical findings, treatments, progress, goals, and discharge plans
• Communicate OASIS scores to the assigned Clinical Team Manager within required timelines
• Supervise Physical Therapy Assistants as needed
• Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
• Perform additional duties consistent with skills, training, and role responsibilities

Coverage Area

Service territory includes San Jose and surrounding areas, including Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, Morgan Hill, San Martin, Gilroy, Sunnyvale, and nearby communities as needed.

Schedule

• Day shifts, approximately 8:00 AM – 5:30 PM
• Schedule flexibility based on patient availability
• One weekend day per month required
• Holiday coverage required

Requirements

• Minimum 1 year of Home Health Physical Therapy experience
• Active California Physical Therapist license
• Ability to work independently in a home-based setting
• Strong clinical assessment and documentation skills

Certifications

• BLS / BCLS (Required)

EMR System

• Homecare Homebase (HCHB)
