About the Role:
We are looking for a compassionate In-Home Services RN to provide skilled nursing care to participants in their homes. You’ll work directly with patients, their families, and the home care team to deliver high-quality, patient-centered care.
Key Responsibilities:
- Provide skilled nursing care in participants’ homes according to physician orders and nursing best practices.
- Assess, monitor, and document participants’ condition, response to treatments, and medication compliance.
- Administer medications and provide education on self-administration of non-scheduled medications.
- Assist with examinations, treatments, and evaluations, and coordinate care with physicians and specialists.
- Schedule appointments, hospital admissions, and follow-up care for participants as needed.
- Educate participants and families on medications, treatments, and health counseling.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and medical social workers to ensure comprehensive care.
- Supervise LPNs, HHAs, and CNAs according to regulatory requirements.
- Complete clinical notes, care summaries, and maintain accurate medical records.
- Participate in care conferences, peer review, quality management, and utilization review.
- Provide preventive and rehabilitative nursing procedures to meet participants’ needs.
Required Qualifications:
- Associate’s degree in nursing (ADN)
- Current state RN license
- Current First Aid and BLS certifications (AHA or Red Cross)
- Reliable transportation, valid driver’s license, good driving record, and auto insurance
- Minimum 1 year experience working with frail or elderly patients
Preferred Qualifications:
- Bachelor’s degree in nursing (BSN)
- Certification as a Gerontological Nurse
- 3+ years of experience in healthcare, preferably geriatrics
- Bi-lingual (Spanish, Russian, Punjabi, or Hmong) is a plus
