Connected Health Care, a trusted healthcare recruitment and staffing firm, is partnering with a reputable Long Term Care / Rehab facility near Springfield, IL to hire a Travel Licensed Practical Nurse (LPN) for a 13-week contract. This opportunity offers steady day shifts, consistent hours, and competitive weekly pay.

Position Details

Position: Travel LPN
Setting: Long Term Care / Rehab
Shift: Day Shift
Schedule: 3x12s
Hours: 36 hours per week
Contract Length: 13 weeks

Job Responsibilities

Provide high-quality nursing care to residents in an LTC/Rehab setting
Administer medications and treatments as ordered
Monitor resident status and report changes to the care team
Accurately document care and treatments in the EMR
Collaborate with RNs, therapy teams, and facility staff
Ensure compliance with facility policies and state regulations
Deliver compassionate, patient-centered care

Education & Certification Requirements

Active Illinois LPN License (or eligibility to work in IL)
Graduate of an accredited Practical Nursing program
Current BLS/CPR Certification
1+ year of LPN experience preferred
LTC or Rehab experience strongly preferred
Ability to pass background check, drug screen, and health requirements
