Licensure requirement-LMSW or LCSW AND MS STATE LICENSE

Must be SW Case manager with at least 2 years Acute Care Management experience. Previous experience in large academic Medical Center hospital/Level 1 trauma center is required

Will cover all Inpatient units throughout the hospital Medical discharge could be some social discharges Case load/# patients per day/consults per day 15-20
