Position Overview

We are seeking a dedicated and adaptable Physical Therapist (PT) for a contract assignment in a skilled nursing facility setting. The PT will be responsible for evaluating patients, developing individualized treatment plans, and delivering therapy services aimed at improving mobility, strength, and overall functional independence.

Key Responsibilities

Evaluate, treat, and monitor patients in a skilled nursing environment
Develop and implement individualized physical therapy treatment plans
Collaborate with the interdisciplinary care team to support optimal patient outcomes
Maintain accurate and timely documentation per facility standards
Float between nearby affiliated facilities as needed

Requirements

Active California Physical Therapist (PT) license
Skilled Nursing Facility (SNF) experience preferred
- Mentorship available for candidates with 0–1 year of experience
Strong documentation, organizational, and time-management skills
Flexible, collaborative, and patient-focused approach to care
Ability to work flexible schedules (Sunday–Thursday, Tuesday–Saturday, or Monday–Friday)
BLS/BCLS certification required
