Job Summary:

The Hospice Registered Nurse (RN) Care Manager is responsible for providing compassionate, patient-centered care to hospice patients in an acute care setting. This role involves coordinating care, managing patient needs, and collaborating with interdisciplinary teams to ensure comfort, dignity, and quality of life for patients and their families.

Roles & Responsibilities:

• Provide comprehensive nursing care to hospice patients in accordance with care plans
• Conduct patient assessments and ongoing evaluations
• Coordinate and manage patient care across the interdisciplinary hospice team
• Educate patients and families on disease progression, symptom management, and end-of-life care
• Participate in on-call coverage (2–3 nights per week) and occasional weekend backup support
• Document patient care accurately using MEDITECH
• Travel to patient homes as required, following agency mileage policies
• Collaborate closely with physicians, case managers, and support staff

• Hospice and end-of-life care expertise
• Strong patient assessment and care coordination skills
• Excellent communication and interpersonal abilities
• Ability to work independently in the field
• Proficiency with electronic medical records (MEDITECH preferred)
• Compassionate and patient-focused approach

Requirements:

• Active RN license of GA State
• Minimum 2 years of nursing experience

• AHA - BLS Certification (required)
