Social Worker

🕒 Full-Time | 40 hours/week
OU Health is seeking a Social Worker II to support the Care Management department on a temporary assignment due to FMLA/LOA coverage. This role provides essential social services to patients and families within an acute care hospital setting.
Position Overview
Under general supervision, the Social Worker II provides comprehensive social services to patients and their families, supporting discharge planning, psychosocial assessments, crisis intervention, and care coordination across the healthcare continuum.
Key Responsibilities
- Provide social services to patients and families for assigned medical services
- Conduct discharge planning evaluations and psychosocial assessments
- Counsel patients and families regarding illness, treatment, emotional stress, and social or financial concerns
- Perform crisis intervention services, including high-risk cases (e.g., abuse or neglect)
- Act as a liaison between patients, families, hospital staff, and community agencies
- Assist with referrals and post-hospital placement or transfer to post-acute care facilities
- Advocate for patients related to informed decision-making, ethical dilemmas, and end-of-life care
- Utilize specialized interviewing and communication skills, including with pediatric patients
- Document services and prepare reports or statistics as needed
- May supervise students or newly hired staff
- Perform other duties as assigned
Qualifications
Education
- Master’s Degree in Social Work from an accredited institution (required)
Licensure
- LMSW strongly preferred
- LMSW or LCSW candidates only
Experience & Skills
- Hospital experience preferred
- Strong knowledge of social work principles, casework practices, and community resources
- Excellent analytical, communication, and interpersonal skills
- Ability to explain complex information clearly to patients, families, and healthcare staff
