Job Summary
We are seeking a Licensed Social Worker (LMSW) for an acute care hospital setting. The Behavioral Health Licensed Social Worker is responsible for assessing patients’ psychosocial needs at admission and throughout their stay, developing and implementing treatment and aftercare plans, and serving as the primary therapist for assigned patients. This role requires close collaboration with patients, families, physicians, and the interdisciplinary care team to ensure comprehensive and coordinated care.
Roles and Responsibilities
- Perform comprehensive psychosocial assessments within required timeframes at admission and on an ongoing basis
- Serve as the primary therapist for assigned behavioral health patients
- Develop, implement, and evaluate individualized treatment and aftercare plans
- Collaborate with patients, families/significant others, physicians, and the interdisciplinary treatment team
- Coordinate and manage discharge planning, including arranging appropriate post-acute services
- Advocate for patients in interactions with hospitals, community organizations, and external agencies
- Identify social, emotional, and environmental factors impacting patient mental health and treatment outcomes
- Document assessments, treatment plans, interventions, and discharge plans in the electronic medical record within established timelines
- Maintain clinical competency, professional ethics, and ongoing professional development
- Participate in special projects, departmental initiatives, and community meetings as requested
Skills
- Strong psychosocial assessment and clinical interviewing skills
- Knowledge of behavioral health treatment planning and discharge coordination
- Effective interdisciplinary collaboration and communication skills
- Strong documentation and organizational abilities
- Patient advocacy and care coordination expertise
- Experience working in acute care or behavioral health settings
- Proficiency with electronic medical records (EPIC preferred)
Requirements
- Active Licensed Master Social Worker (LMSW) license required
- Minimum 2 years of social work experience, preferably in an acute or behavioral health setting
- AHA - BLS certification required
- Master Degree in Education
- Experience with discharge planning and care coordination
Ability to work day shifts, 8-hour shifts
