Description

Job Title: Licensed Clinical Social Worker (LCSW)
Job Type: Full-Time Hybrid (Remote and In-Office)
Location: Mount Vernon, NY
 

Job Description:
We are looking for a Licensed Clinical Social Worker (LCSW) to join our team. In this role, you will support individuals through counseling and care coordination in both individual and group settings. Your main duties include:

  • Meeting with individuals to understand their needs and personal history.
  • Creating and following treatment plans to support emotional and mental health.
  • Tracking progress and sharing updates with the care team.
  • Referring individuals to other healthcare providers or support programs when needed.
  • Completing crisis assessments and providing appropriate support.
  • Offering wellness-related services as part of integrated care.
  • Keeping clear and accurate documentation.

Requirements:

  • Master’s degree in Social Work or a related field.
  • Active LCSW license in New York State.
  • 1–2 years of experience in mental health services.
  • Bilingual English/Spanish is a plus.
  • Ability to work in a hybrid setting.
