Position Overview

A busy, well-established outpatient clinic in San Jose is seeking a Family Nurse Practitioner (FNP) to provide comprehensive primary care services to patients across the lifespan. This is a 100% ambulatory role focused on preventive care, chronic disease management, and continuity of care.

The ideal candidate is comfortable working in a fast-paced clinic environment and values patient-centered, team-based practice.

Practice Setting

Key Responsibilities

• Provide primary care for pediatric, adult, and geriatric populations
• Perform physical exams, assessments, and diagnoses
• Develop and manage treatment plans
• Manage chronic diseases (diabetes, hypertension, asthma, etc.)
• Order and interpret labs and diagnostic studies
• Prescribe medications according to California NP regulations
• Conduct preventive screenings and annual wellness visits
• Educate patients on lifestyle, diet, and disease prevention
• Maintain timely and accurate EMR documentation
• Coordinate referrals and follow-up care

Qualifications

• Certified Family Nurse Practitioner (FNP)
• Active or eligible for California NP license
• DEA (or ability to obtain)
• Outpatient / ambulatory clinic experience required
• Strong communication and patient engagement skills
