Description

Requirements:
• RN License (WA or Compact)
• Minimum 2 years acute care hospital experience
• Case Management / Discharge Planning experience required
• BLS required (ACLS, PALS preferred)
• Ability to float within hospital as needed

Role Highlights: • Acute care RN Case Management position • Focus on care coordination, discharge planning, DME needs, and plan of care • Works across all hospital units and both campuses • Strong interdisciplinary collaboration with Social Services • Charting: EPIC
