Position Overview

We are seeking a licensed Occupational Therapist (OT) for a 13-week contract assignment in a Skilled Nursing Facility / Long-Term Care setting. The OT will be responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions to improve independence, safety, and quality of life.

Key Responsibilities

Evaluate patients’ functional abilities related to activities of daily living (ADLs)
Develop and implement individualized occupational therapy treatment plans
Provide therapeutic interventions to improve strength, coordination, mobility, and cognitive function
Train patients in the use of adaptive equipment and techniques
Monitor patient progress and modify treatment plans as appropriate
Maintain accurate and timely clinical documentation
Collaborate with interdisciplinary healthcare teams to support patient-centered goals
Educate patients, families, and caregivers on therapy strategies and safety

Qualifications & Requirements

Active State Occupational Therapist license (must be current)
Minimum of 1 year of experience as an Occupational Therapist
Experience in Skilled Nursing Facility or Long-Term Care settings preferred
Strong clinical assessment, communication, and documentation skills
