Job Title: Home Health Physical Therapist

Setting: Skilled Home Health (Medicare, Medicare Advantage & Commercial Insurance)
 

Job Summary

We are seeking an experienced Home Health Physical Therapist to provide skilled therapy services to patients in their homes. The therapist will evaluate, treat, and educate patients to improve mobility, functional independence, and overall quality of life while ensuring accurate clinical documentation and compliance with home health standards.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized treatment plans
• Provide therapy for patients with med/surg, cardiac, orthopedic, neurological, respiratory, and post-surgical conditions
• Treat patients with conditions such as COPD, CHF, CVA, Parkinson’s, trauma, falls, weakness, amputations (AKA/BKA), and failure to thrive
• Implement and monitor Home Exercise Programs (HEP)
• Educate patients and families on safety, injury prevention, and care plans
• Utilize and educate on equipment: gait belts, walkers, wheelchairs, oxygen, sit-to-stand lifts, sliding boards, collars, prosthetics, etc.
• Complete accurate clinical documentation including OASIS (preferred)
• Recognize and report signs/symptoms of physical, psychological, or sexual abuse
• Ensure compliance with billing, documentation, and regulatory requirements

Required Skills & Experience

• Minimum 1 year of Physical Therapy experience
• Home Health experience required
• Strong skills in:
  • Assessment & documentation
  • Cardiopulmonary/circulatory care
  • Ortho & Neuro rehab
  • Patient & family education
  • Safety & injury prevention

Certifications & Licensure

• Active State Physical Therapist License
• BLS Certification (required)

Additional Requirements

• Must have valid driver’s license and reliable vehicle for home visits
• COVID vaccination or approved exemption required
• Flu vaccination or declination (mask mandatory if declined)
• Willing to work 1–2 weekends per contract
• Scrubs: Any solid color
