Travel LPN – Home Health / Outpatient

• Shift: 5×8 Days | 8:00a–5:00p, Mon–Fri
• Hours: 40 hrs/week (Guaranteed)
• Contract Length: 8–13 weeks
• Region: Central (Baytown area; ~30–40 mile radius)
• Setting: Outpatient Home Health (20% visits in Assisted Living Facilities)

Requirements:

• Experience: Minimum 1 year LPN, including 6 months Home Health (private duty not accepted)
• Certifications: BLS / First Aid (AHA or ARC)
• Charting: WellSky / OASIS
• Licensure: Active LPN license
• Must attend daily 8:15 AM conference call
