Position Overview
We are seeking a licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) setting. The Physical Therapist will evaluate residents, develop individualized care plans, and deliver evidence-based therapeutic interventions to improve mobility, strength, and functional independence.
Key Responsibilities
- Perform comprehensive physical therapy evaluations
- Develop and implement individualized treatment plans
- Provide therapeutic exercises, gait training, balance training, and mobility interventions
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on home exercise programs and safety strategies
- Maintain accurate and timely documentation of services provided
- Collaborate with an interdisciplinary team to support patient-centered goals
- Ensure compliance with facility policies and regulatory standards
Qualifications & Requirements
- Active State Physical Therapist license (must be current)
- Experience in a Skilled Nursing Facility preferred
- Candidates with less than 1 year of experience may be considered
- Strong clinical decision-making and documentation skills
- Ability to work independently and as part of a rehabilitation team
