Description

Travel LMSW (Licensed Master Social Worker) – Acute Care Case Management

Everett, WA

Duration: 13 Weeks
Shift: Days | 5x8 (07:30 AM – 4:00 PM)

 Charting: EPIC

Guaranteed Hours: 72 hours per 2 weeks
Weekend Requirement: Every other weekend

 

 

Key Responsibilities

  • Perform biopsychosocial assessments and initial patient evaluations
  • Lead discharge planning and care coordination
  • Coordinate home care, DME, rehab, and long-term care referrals
  • Support patients and families with advance directives, guardianship, and protective services
  • Collaborate with multidisciplinary teams to ensure cost-effective and safe transitions of care
  • Assist with Medicaid, Medicare, medical transportation, and medication assistance
  • Document care plans, progress notes, and discharge plans in EPIC
  • Manage an average caseload of 18–24 patients (up to 30 on weekends)
  • Float across hospital units as needed

Required Qualifications

  • MSW or LCSW (Master’s Degree Required)
  • Active Washington Social Work License (LICSW, LSWAIC, LASW, or LSWAA accepted)
  • 1+ year of Acute Care Social Work or Case Management experience
  • Strong experience in Discharge Planning & Care Coordination
  • EPIC charting experience required
  • Knowledge of community resources, insurance programs, and protective services
  • BLS Certification

Preferred Experience

  • Experience in Med-Surg, ED, ICU, LLOS, or Mother-Baby/NICU
  • Strong skills in high-risk patient intervention and cultural competency
  • Background in acute hospital social work
