Will be all 8 hour days but may also be expected to cross cover adult home health, hospice and community hospice as needed.Requirements: Education: Master’s Degree in Social Work (MSW), Experience: 2+ years of relevant experience, Licensure: Vermont LMSW required. EMR is netsmart, converting to Epic. Epic experience is required in the past 24 months.
