In-Home Services RN – Loveland, CO

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality in-home nursing care to clients in Loveland, CO. In this role, you will deliver exceptional clinical care in the comfort of clients’ residences while completing essential documentation and care coordination responsibilities.

What You’ll Do

• Provide skilled nursing care in clients’ homes, including medication administration and treatments
• Perform ongoing assessments and reassessments to ensure safe, effective care
• Implement physician orders and individualized care plans
• Educate clients and families on medications, treatments, and self-care strategies
• Coordinate outside appointments and assist with hospital admissions as needed
• Observe, document, and report client conditions and responses to treatment
• Complete clinical documentation including progress notes, discharge summaries, and care conference reports
• Collaborate with interdisciplinary team members (PT, OT, SLP, Social Worker)
• Supervise LPNs and HHAs/CNAs in accordance with regulatory standards
• Participate in quality improvement, peer review, and utilization review activities

Requirements

• Associate’s Degree in Nursing
• Active Colorado RN license
• Current First Aid and BLS certification
• Reliable transportation, valid driver’s license, clean driving record, and auto insurance
• Minimum of 1 year experience working with elderly or medically frail populations

Preferred Qualifications

• Bachelor’s Degree in Nursing
• Gerontological Nursing certification
• 3+ years of healthcare experience with an emphasis in geriatrics
• Bilingual (Spanish, Russian, Punjabi, Hmong) strongly preferred
