Position Overview

We are seeking a licensed Physical Therapist for a contract assignment in a Skilled Nursing Facility setting. The therapist will evaluate and treat patients to improve mobility, restore function, reduce pain, and prevent disability following injury, illness, or surgery.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans
• Implement therapeutic exercises and rehabilitation programs
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy techniques and discharge planning
• Collaborate with interdisciplinary healthcare team members
• Maintain accurate and timely clinical documentation

Minimum Qualifications

• Active state Physical Therapist license (must be current)
• Experience in a Skilled Nursing Facility preferred
• New graduates considered (less than 1 year experience accepted)
• Strong communication and patient care skills
