Description

Facility Name: Orleans Essex Visiting Nurses Association & Hospice, Inc.
Title: LPN – Home Health (OASIS Required)
Location: Newport, VT (Upper Vermont Territory)
Position Type: 13 Weeks (36 Hours Guaranteed)
Shift: Day Shift – Monday–Friday (08:00 AM – 05:00 PM)
Weekly Pay Package:

W2 Wage (RT+OT based on expected hrs): $600.00

Nontaxable Weekly PerDiem: $1,200.00

Total Weekly Gross Pay Package: $1,800.00

Job Description:
• License: Active Vermont LPN license required
• Certifications: BLS required
• Experience: Minimum 2 years Home Health experience required; Hospice experience preferred
• OASIS experience required
• Experience traveling between patient homes for visits required
• Must be able to manage schedule changes and adjust to daily routing
• Valid driver’s license in good standing required
• Provides skilled nursing care in home health and hospice settings
• Performs wound care, medication administration, patient education, and documentation
• EMR system used: HealthWyse (Careficient)
• Some weekend coverage required
• Emergency call rotation may be scheduled

Additional Notes: • Holiday coverage required • Mileage reimbursed at $0.50 per mile between appointments (details confirmed during interview) • Facility conducts its own background check
