Description
Job Summary
Under the direct supervision of a Registered Nurse or physician, the Licensed Practical Nurse (LPN) provides nursing care to patients on an assigned unit. The LPN delivers safe, effective, and patient-centered care using sound clinical judgment and established nursing practices. This role requires strong clinical skills, accurate documentation, and the ability to collaborate with interdisciplinary teams to meet patient care goals.
Key Responsibilities
- Perform nursing duties in the care and treatment of assigned patients
- Conduct patient assessments using diagnostic data and clinical observations
- Monitor patient conditions and report significant changes promptly
- Contribute to individualized plans of care with measurable goals
- Provide ongoing patient care based on clinical priorities and needs
- Perform timely and accurate documentation in patient records
- Reassess patients at regular intervals and as condition changes
- Maintain patient privacy and dignity during care
- Collaborate with physicians, RNs, Care Managers, and clinical staff
- Assist with discharge planning and post-care coordination
- Supervise and support non-professional staff as assigned
- Follow all facility policies, safety protocols, and infection control standards
Required Qualifications
Education
- Graduation from an accredited Practical Nursing Program
OR - Equivalent education from an approved professional nursing program
Experience
- Minimum 1 year of clinical experience preferred
License / Certification
- Current New York State LPN License (Required)

Physical & Work Environment Requirements
- Ability to stand, walk, bend, stretch, and lift patients
- Ability to sit for extended periods for documentation
- Exposure to blood, body fluids, and tissue
- Use of PPE and adherence to infection control protocols
- May be exposed to mechanical, electrical, chemical, and biological hazards
- Ability to safely handle medical equipment and hazardous materials
