Position Overview

York Hospital is seeking an experienced RN Care Manager for a 13-week contract assignment at its main campus in York, Maine. This is a 40-hour per week position with variable scheduling options and no on-call requirement.

Assignment Details

• Start Date: March 23, 2026
• Duration: 13 Weeks
• Schedule Options:
  • 8:30 AM – 4:30 PM (5x8s)
  • 7:30 AM – 5:00 PM (4x10s)
• Call: None
• Weekend & Holiday Rotation: Required
• Guaranteed Hours: 40 per week

Key Responsibilities

• Coordinate concurrent utilization management activities hospital-wide
• Serve as the primary liaison between patient, care team, organization, and payors
• Conduct comprehensive patient assessments and identify transition-of-care needs
• Secure community resources including DME, insurance authorizations, and transportation
• Lead and collaborate in interdisciplinary rounds and care planning meetings
• Monitor length of stay and support appropriate resource utilization
• Maintain strong documentation and communication across electronic systems

Required Qualifications

• Active Registered Nurse (RN) license (must be current)
• Associate or Bachelor’s Degree in Nursing
• Minimum 3–5 years clinical experience in:
  • Medical/Surgical Nursing
  • Home Care
  • Hospital Case Management
  • Utilization Review
• Required Experience: Utilization Review and Discharge Planning
• BLS Certification (Active)
