Job Summary:

We are seeking a Licensed Master Social Worker (LMSW) to provide behavioral health services in an acute care setting. The Social Worker will assess patients’ psychosocial needs upon admission and throughout their stay, develop individualized treatment and aftercare plans, and serve as the primary therapist for assigned patients. The role requires strong collaboration with interdisciplinary team members to ensure comprehensive patient-centered care and appropriate discharge planning.

Roles and Responsibilities:

• Conduct comprehensive psychosocial assessments within required timeframes upon admission and ongoing thereafter.
• Serve as the primary therapist for assigned patients.
• Develop, implement, and evaluate individualized treatment and aftercare plans in collaboration with patients, families, and the interdisciplinary care team.
• Collaborate with physicians and healthcare team members to address significant social and emotional factors impacting mental health.
• Monitor patient progress and revise treatment plans as necessary.
• Coordinate and facilitate discharge planning, ensuring appropriate referrals and continuity of care.
• Advocate for patients in interactions with hospitals, agencies, and community organizations.
• Maintain accurate and timely documentation of all treatment phases in the electronic medical record (EPIC).

Requirements:

• Minimum 2 years of Social Work experience in an acute or behavioral health setting.
• Active New York LMSW License
• AHA - BLS Certification
