Description

Experience: Provider must have a minimum of 24 months of current Emergency Room experience (REQUIRED).
Certifications: Must be issued by the American Heart Association or American Red Cross (REQUIRED).
License Verification: License check must show no hits at the time of submission (REQUIRED – documentation must be included).
Flu Vaccination: Required. Banner declination forms are available upon request. Please include vaccination status in the submission notes.
Travel Experience: Preferred.
