Description
We are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services to patients in the Grand Junction.
The Physical Therapist will deliver skilled physical therapy services in patients’ homes in accordance with physician orders, payer requirements, agency policies, and applicable state and federal regulations.
This role supports patient mobility, safety, and functional independence through individualized, evidence-based care.
Clinical Responsibilities
- Perform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits
- Develop and implement individualized physical therapy plans of care based on patient needs and physician orders
- Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention
- Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
- Communicate patient progress, concerns, or changes in condition to the agency’s interdisciplinary care team
- Collaborate in the clinical oversight of Physical Therapist Assistants (PTAs), as applicable and permitted under Colorado law and agency policy.
Documentation & Systems
- Complete all required documentation accurately and timely in Homecare Homebase (HCHB)
- Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards
- Maintain compliance with HIPAA and all applicable privacy and security requirements
Care Coordination
- Collaborate with agency clinical staff, including RNs, PTAs (when applicable), OTs, STs, and leadership
- Participate in care coordination related to patient needs, visit completion, and regulatory requirements
- Provide clinical oversight of PTAs only when applicable, assigned, and permitted under Colorado law and agency policy
Qualifications
Required
- Current, unrestricted Colorado Physical Therapist license
- Graduation from an accredited Physical Therapy program
- Valid driver’s license and reliable transportation
- Ability to practice independently in a home-based setting
- Proficiency or willingness to learn Homecare Homebase (HCHB)
Preferred
- Home health experience
- Familiarity with Medicare documentation and home health regulations
- Experience working with geriatric and medically complex populations
Practice Environment
