Description
Fast Paced Building with hi volume of evaluations.
Must have confirmed SNF post graduate experience as well as PCC/NetHealth proficiency
Responsibilities:
Evaluate patients’ functional abilities and develop individualized treatment plans
Provide hands-on therapy in accordance with physician orders and facility protocols
Document patient progress and maintain accurate treatment records
Collaborate with physical therapists, speech-language pathologists, nurses, and other healthcare professionals
Educate patients, families, and caregivers on techniques to support continued care and safety
Ensure compliance with all state and federal regulations
Qualifications:
Current Occupational Therapist license in the state
Graduate of an accredited Occupational Therapy program
Strong interpersonal and communication skills
Ability to work independently and as part of a team
BLS certification
