Description

Job Summary:

Under general supervision, provides social services to patients and families. Responsible for psychosocial assessments, discharge planning, crisis intervention, and coordination of post-hospital care.


Responsibilities:

  • Provide social services for assigned medical unit
  • Conduct psychosocial assessments and discharge planning
  • Counsel patients and families on illness, treatment, and social or financial concerns
  • Provide crisis intervention and high-risk case support (abuse, neglect, etc.)
  • Act as liaison between patients, families, hospital staff, and community agencies
  • Assist with post-acute placement and community referrals
  • Advocate for patients regarding care decisions and ethical concerns
  • Support patients and families adjusting to illness or functional changes
  • May supervise students or new staff
  • Perform other duties as assigned

Requirements:

  • Master’s Degree in Social Work (MSW) from accredited program
  • Hospital experience preferred
  • LMSW strongly preferred
  • Strong assessment, analytical, and communication skills
  • Knowledge of community resources and case management practices
