NP – Hospice

Location: Fresno, California
Duration: 13 Weeks
Start Date: March 9, 2026
Shift: Day Shift (Monday–Friday) 8/5
Weekly Pay: $3,665

Position Overview
Seeking a Hospice Nurse Practitioner for a part-time travel assignment with pediatric hospice experience. This role involves home-based hospice visits, coordination with skilled nursing facilities (SNFs), and collaboration with the Medical Director for Medicare recertifications.

Key Responsibilities

Conduct 4–5 hospice home visits per day
See patients in SNFs and other care facilities as needed
Perform face-to-face evaluations for Medicare recertifications
Communicate with Case Managers regarding patients requiring follow-up
Round in the inpatient unit as needed
Provide education to clinical team members
Flexible schedule; full or half days can be arranged

Requirements

Advanced Practice license (NP)
Pediatric hospice experience preferred
Excellent clinical assessment and communication skills
Ability to manage patient care independently
