Location: Fresno, California
Duration: 13 Weeks

Shift: Day Shift (Monday–Friday) 8/5
Weekly Pay: $3,665
Position Overview
Seeking a Hospice Nurse Practitioner for a part-time travel assignment with pediatric hospice experience. This role involves home-based hospice visits, coordination with skilled nursing facilities (SNFs), and collaboration with the Medical Director for Medicare recertifications.
Key Responsibilities
- Conduct 4–5 hospice home visits per day
- See patients in SNFs and other care facilities as needed
- Perform face-to-face evaluations for Medicare recertifications
- Communicate with Case Managers regarding patients requiring follow-up
- Round in the inpatient unit as needed
- Provide education to clinical team members
- Flexible schedule; full or half days can be arranged
Requirements
- Advanced Practice license (NP)
- Pediatric hospice experience preferred
- Excellent clinical assessment and communication skills
- Ability to manage patient care independently
