Travel LTC LPN (Licensed Practical Nurse) – Full-time (13 weeks)
Location: Barron, WI (Care And Rehab – Barron)
Start: 03/23/2026 | Schedule: Evenings 2:00 PM–10:30 PM, 40 hrs/week, every other weekend | On-site | Housing: Not provided
Connected Health Care is seeking an LPN for a long-term care (LTC/SNF) assignment. You’ll provide direct resident care, administer medications/treatments, monitor and document vital signs in the EMR, and collaborate with the care team to support individualized care plans.
Requirements
- Active, unencumbered Wisconsin LPN license
- 1+ year recent LTC experience
- BLS/CPR + First Aid
- Ability to pass background check; strong communication skills
