Job Description

• Night shift: 7:00 PM to 7:30 AM
• Specialty: Medical/Surgical and Telemetry nursing

Required Qualifications

Experience

• Specialty: RN Medical/Surgical/Telemetry
• Required years of experience: Not explicitly stated; assume standard RN experience for Med/Surg/Telemetry roles (typically 1+ years)

Licensure

• Must hold a valid RN license in Ohio
• License verification must be dated within 30 days of start

Certifications

• BLS (Basic Life Support) certification required
• ACLS (Advanced Cardiovascular Life Support) certification required for RNs in critical care areas (applicable here due to telemetry specialty)

Immunizations & Health Requirements

• COVID-19 vaccine proof (including booster through September 2025) or approved exemption/declination (declination not accepted for CA or IL)
• MMR vaccination proof or immunity
• Hepatitis B vaccination series required (titers not accepted)
• Influenza vaccine proof or exemption for the current flu season
• Varicella vaccination proof or immunity
• 2 Step PPD or IGRA (Quantiferon/Tspot) for TB screening
• Physical/Health statement confirming fitness to work (within last year)
• Drug screen: 10-panel drug screen completed within 30 days of start (NO THC)

Background & Compliance

• Federal, nationwide, and county criminal background check within last 30 days
• Social Security Number verification
• Education verification (highest level of academic education)
• Exclusion screenings (EPLS, OIG/GSA) within last 30 days
• Conflict of Interest Disclosure and Confidentiality Agreements
• Integrity and Audit Services Form
• Trinity Joint Commission Testing (CORE Exam) within last year
• National Sex Offender Registry check within last 30 days
• System Access Form completed one week before start
• Skills Checklist within last year
• Specialty Specific Competency Testing within last year
