Connected Health Care is seeking a Travel Physical Therapist for a Skilled Nursing Facility (SNF) assignment in DeSoto, Texas (75115). This is an on-site, day-shift contract with consistent weekday scheduling.

What you’ll do

Provide PT evaluations and treatment plans for SNF patients
Deliver skilled therapy services to improve mobility, strength, safety, and functional outcomes
Document care and progress per facility standards and compliance guidelines
Collaborate with the interdisciplinary rehab/team to support patient goals and discharge planning

Requirements (Must-Haves)

Active Texas PT License
BLS
SNF experience
Minimum 1 year experience required
