Submission Requirements
· Must have a minimum of 24 months Endoscopy experience (with recent experience in last 12 months) - [REQUIRED]
· Life support certifications must be from AHA, ARC, or ASHI - [REQUIRED]
· No hits on state license to practice - [REQUIRED] [DOCUMENT]
· RTO cannot exceed 7 days, No RTO during the first week of assignment - [REQUIRED]
· Candidate driver’s license to verify residency - [REQUIRED]
· Flu Vaccination: Medical and/or religious exemption forms available upon request. Please include vaccination status in submission notes.
· Travel experience - [HIGHLY PREFERRED]
