RN Outpatient / Home Health | 5x8 Days | Travel

Shift & Schedule

5x8 Days: 8:00 AM – 5:00 PM, Monday–Friday
Weekend/Holiday work: Only if necessary for patient care, requires supervisor approval
Guaranteed Hours: 40/week
Contract Length: 8–13 weeks

Facility & Setting

Region: Central (Baytown, 30–40 mile radius)
Setting: Outpatient, with ~20% of visits in Assisted Living Facilities
Mileage: GSA Maximum

Requirements & Certifications

Licensure: RN
Required Certifications: BLS / First Aid (AHA or ARC)
Experience:
- Minimum 1 year recent LPN experience
- At least 6 months in a home health setting
- Private duty experience not accepted

Responsibilities

Attend daily conference call at 8:15 AM
Provide nursing care in outpatient and assisted living settings
Chart patient care in WellSky / OASIS
