Physical Therapist (PT) – 13-Week Contract

Day Shift | 40 Hours per Week

Position Summary

The Physical Therapist partners with residents and interdisciplinary care providers to deliver high-quality physical therapy services that maximize residents’ functional abilities and independence in activities of daily living. This role supports a resident-centered approach to care while promoting the mission and values of Covenant Health Ministry.

This is a 13-week contract position working day shift, 40 hours per week.

Essential Duties and Responsibilities

  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Performs evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider, observing all precautions, indications, and contraindications.
  • Evaluates residents per department protocols and establishes individualized plans of care, including functional short- and long-term goals, frequency and duration of services, and appropriate therapeutic interventions.
  • Collaborates with residents and primary caregivers to incorporate their input into therapy goals and care planning.
  • Provides consultation to referring physicians and interdisciplinary team members regarding care plans and discharge needs; attends pertinent care team meetings.
  • Monitors treatment effectiveness, progresses therapy programs toward established goals, and modifies plans of care as necessary.
  • Documents all evaluative and therapeutic interactions in the Electronic Medical Record (EMR) according to department procedures, ensuring clarity and accessibility for the care team.
  • Recommends and obtains appropriate assistive devices and ensures residents and caregivers receive instruction in proper use.
  • Provides education to residents and caregivers and documents outcomes of teaching.
  • Refers residents appropriately to the Physical Therapist Assistant (PTA) and collaborates to ensure high-quality resident care.
  • Maintains compliance with Medicare, Medicaid, private pay, and other reimbursement regulations.
  • Provides in-service education to nursing and other departments; maintains current clinical knowledge and skills through continuing education.
  • Assists with the development of new rehabilitation programs and services.
  • Maintains awareness of professional issues and communicates pertinent updates to staff.
  • Supervises assistant-level staff as appropriate, ensuring adherence to state mandates and professional standards. Reports performance concerns to the Director.
  • Mentors new staff during orientation and ensures completion of required competencies.
  • Maintains department cleanliness and equipment per infection control and system standards.
  • Maintains required licenses, certifications, competencies, and completes annual compliance requirements.
  • Performs other duties consistent with this role.

Job Requirements

Education and Experience

  • Bachelor’s degree required; Master’s or Doctorate in Physical Therapy preferred
  • Active Physical Therapist (PT) license in the applicable state
  • Basic Life Support (BLS) certification preferred
  • Prior experience in a long-term care or skilled nursing facility preferred
