Description
Position: Nurse Coordinator – Post-Lung Transplant
Shift: 08:00 – 16:30
Requirements:
- Active RN license
- Minimum 3 years of ICU RN experience
- Strong critical care background preferred
- BLS (ACLS preferred if applicable)
General Summary / Overview:
The Post-Lung Transplant Coordinator reports directly to the Medical Director of the Lung Transplant Program with administrative oversight by the Transplant Center Director (or designee). This role ensures continuity of care following hospital discharge and serves as the primary point of contact for patients enrolled in the lung transplant program.
The scope includes patient care, education, quality assurance, research support, administrative coordination, financial navigation, and collaboration with medical and surgical teams to optimize transplant outcomes.
Patient Care – Post-Transplant Discharge Phase
- Provides outpatient care within RN scope of practice to support optimal physical, emotional, and social rehabilitation.
- Assesses and addresses educational, psychosocial, and financial needs of recipients and families.
- Reinforces medication adherence and post-transplant follow-up requirements.
- Monitors compliance with immunosuppressive regimens and collaborates with transplant team to ensure adherence.
- Collects, reviews, and documents clinical data related to organ function and overall health status.
- Coordinates laboratory testing and diagnostics for clinic visits and communicates results to transplant physicians.
- Implements medication and treatment adjustments as ordered by credentialed providers.
- Serves as a resource regarding post-transplant protocols (e.g., high-risk labs/NAT testing, rejection treatment, outpatient monitoring schedules).
- Arranges consultations, procedures, and admissions when indicated (bronchoscopies, thoracenteses, sleep studies, IV infusions, ECP treatments, ED referrals, direct admits, insurance authorizations).
- Maintains communication with referring physicians, PCPs, and specialty providers.
- Ensures comprehensive and accurate EMR documentation.
