Job Description

We are seeking a dedicated Physical Therapist (PT) for a Home Health field position in the Central Valley/Stockton territory. The Physical Therapist will provide skilled therapy services to patients in their homes, focusing on improving mobility, strength, and functional independence.

This is a field-based role requiring travel between patient homes.

Responsibilities

Provide physical therapy services in a home health setting
Evaluate and develop individualized treatment plans
Monitor patient progress and adjust treatment plans as needed
Educate patients and families on therapy programs and safety
Complete accurate and timely clinical documentation
Coordinate care with interdisciplinary healthcare teams
Maintain compliance with home health regulations
Travel between patient homes within assigned territory

Requirements

Active California Physical Therapist License preferred (Pending license accepted – must have applied)
Minimum Home Health experience preferred
OASIS documentation experience required
- Candidates without OASIS may be considered if they have strong Home Health experience
Current BLS Certification preferred
Valid driver’s license and reliable transportation required
