Description
Position: Nurse Practitioner / Physician Assistant
Employment Type: Full-Time
Compensation: $159,000 – $172,000 annually
Role Overview
Provide comprehensive primary care services within an integrated health center model, emphasizing prevention, chronic condition management, behavioral health collaboration, and community-based care delivery.
Key Responsibilities
• Perform comprehensive assessments, diagnoses, and treatment planning
• Prescribe and manage medications within scope of practice
• Deliver preventive care, primary care, and ongoing health maintenance
• Conduct physical exams, review vitals, and evaluate medical histories
• Educate patients on disease processes, medications, prevention, and adherence
• Collaborate with interdisciplinary teams including behavioral health and wellness staff
• Refer patients to subspecialists as appropriate
• Participate in after-hours on-call rotation
• Maintain accurate and compliant clinical documentation
• Engage in team meetings, peer reviews, and quality improvement initiatives
• Assist with coverage for other clinicians as needed
Qualifications
Required:
• Master’s degree or higher as a Nurse Practitioner or Physician Assistant
• Active California NP or PA license
• Current DEA registration
• Minimum 2 years of clinical experience
• Clinical background in Family Medicine, Internal Medicine, or Pediatrics
• Bilingual proficiency required in at least one of the following: Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese
Preferred:
• Experience in community health or integrated care settings
• Knowledge of local, state, and federal healthcare regulations
• Strong analytical, critical-thinking, and communication skills
• Ability to work effectively within a multidisciplinary team
