JOB DESCRIPTION:
Position - RN-Home Health
Location - 677 Church St, Marietta, GA 30060

JOB DETAILS:
Full Time - 13 weeks contract
Day Shift - Mon-Fri 8:30 am to 5 pm// 40 hours weekly
Work Location - On Location
Ability to commute/relocate: Marietta, GA 30060: Reliably commute or planning to relocate before starting work (Preferred).

RESPONSIBILITIES:
Conducted comprehensive in-home patient assessments and developed individualized care plans
Administered medications, IV therapy, injections, and wound care per physician orders
Monitored vital signs and evaluated patient progress, reporting changes to providers
Educated patients and families on disease management, medications, and post-hospital care
Managed chronic conditions such as CHF, COPD, diabetes, and post-surgical recovery
Performed OASIS assessments and maintained accurate documentation in EMR systems
Coordinated care with physicians, therapists, social workers, and interdisciplinary teams
Ensured compliance with Medicare, Medicaid, and home health regulations
Promoted patient safety, fall prevention, and infection control in the home setting
Assisted with care transitions to reduce hospital readmissions

REQUIREMENT:
RN License

Minimum 2+ years of experience
BLS
