Position: Home Health Licensed Practical Nurse (LPN)
Location: Greenwich, Connecticut
Contract Length: 13 weeks
Shift: 5×8 Days (8:30 AM – 5:00 PM)
Weekend Rotation: Alternating weekends
Hours Guaranteed: 40 hours weekly
Housing: Not provided
License: Compact LPN or CT single‑state LPN license in hand

📝 Job Description

We are seeking an experienced Home Health LPN for a 13‑week assignment in Greenwich, CT. This role involves day‑shift home health visits with alternating weekend rotations. Candidates must be dependable, organized, and clinically confident in managing home‑based patient care.

⭐ Position Requirements

Minimum 2 years of Home Health experience
Experience with Homecare Homebase/OASIS documentation
WellSky EMR and wound‑care experience (preferred but not required)
Experience with on‑call/triage
Strong time‑management skills and reliability
Professional, compassionate, and committed to excellent patient care
Active Compact or CT LPN license in hand at time of submission

📞 On‑Call Expectations

1–2 weeks per month
Weekdays 5:30 PM – 8:30 AM
Primarily phone triage; in‑person visits are rare

📊 Productivity Requirements (30 Points/Week)

Revisit = 1 point
Start of Care (SOC) = 2 points
ROC = 2 points
Recert = 1.5 points
OASIS discharge = 1.5 points
