Job Summary:

We are seeking a dedicated and compassionate Occupational Therapist (OT) for a 13-week contract assignment in a Skilled Nursing Facility setting. The ideal candidate will provide high-quality rehabilitative care to patients, helping them regain independence and improve daily functional abilities.

Key Responsibilities:

• Evaluate and assess patients’ physical, cognitive, and emotional abilities
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to improve activities of daily living (ADLs)
• Collaborate with interdisciplinary team members to coordinate patient care
• Educate patients and families on treatment plans and home exercise programs
• Maintain accurate and timely documentation of services provided

Qualifications:

• Active and current state Occupational Therapist license
• Experience in a Skilled Nursing Facility preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• BLS certification (if required by facility)
