Job Title: In-Home Services Registered Nurse (RN)
Location: Aurora, CO
Schedule: Sunday – Thursday | 7:00 AM – 3:30 PM

Overview
We are seeking a compassionate Registered Nurse (RN) to provide skilled nursing care to participants in their homes and support care coordination within an interdisciplinary care team. The In-Home Services RN delivers clinical services in accordance with InnovAge policies and applicable regulations while ensuring participants receive high-quality care in the least restrictive environment.
This position requires travel to participant homes and InnovAge facilities as needed.
Responsibilities
- Triage participant and staff phone calls to determine priority of care and route appropriately.
- Schedule outside appointments for participants when necessary.
- Assist in the examination, testing, and treatment of participants.
- Implement orders written by primary care physicians.
- Complete certification/recertification orders and discharge summaries.
- Instruct participants and families regarding medications and treatment instructions.
- Schedule specialist and other appointments and assist with hospital admissions when necessary.
- Perform general nursing care including administering prescribed medications and treatments.
- Document all necessary information and maintain participant medical records in accordance with agency policies.
- Administer medications, train participants in self-administration of non-scheduled medications, and monitor medication compliance.
- Provide health counseling to participants and families on an individual or group basis.
- Observe, record, and report participant conditions and reactions to treatments to physicians.
- Perform nursing care in participants’ homes.
- Assess and reassess participants to determine care needs and ensure the least restrictive environment.
- Identify changes in condition requiring a change in level of care and coordinate plans with Home Care resources.
- Initiate preventive and rehabilitative nursing procedures.
- Prepare clinical and progress notes for each client visit and document care conferences.
- Conduct client care conferences for assigned participants.
- Participate in peer review, quality management, and utilization review as assigned.
- Process orders and notify physicians of participant needs or condition changes.
- Determine the amount and type of nursing care needed by each client.
- Ensure quality care delivery through care plan review, participant/caregiver communication, and data review.
- Refer participants to Physical Therapy, Occupational Therapy, Speech Language Pathology, or Medical Social Work as appropriate.
- Supervise LPNs every 30 days and HHA/CNAs every 14 days (or per regulatory requirements).
- Train and mentor other nursing personnel as needed.
- Perform additional duties as assigned.
Required Qualifications
- Associate’s Degree in Nursing
- Active State Registered Nurse (RN) License
- Current BLS and First Aid certification (American Heart Association or American Red Cross)
- Valid driver’s license, reliable transportation, and current auto insurance
- Minimum 1+ year experience working with frail or elderly populations
Preferred Qualifications
- Bachelor’s Degree in Nursing (BSN)
- Certification as a Gerontological Nurse
- 3+ years healthcare experience, with emphasis in geriatrics
- Bilingual candidates encouraged to apply (Spanish, Russian, Punjabi, or Hmong)
