Description

Home Health RN Needed

Shift: 8H Days
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Productivity: 6 points per day (SOC and revisits)
Guaranteed Hours: 40/week
Contract Length: 13 weeks
Scrubs: Royal Blue
Charting System: EPIC
License to Submit: Yes (facility willing to wait)

Patient Population / Diagnoses

Post COVID pneumonia
Wound care patients
IV antibiotics
CHF exacerbation
COPD exacerbation
Post-op joint replacement
Mostly adult patients, occasional pediatrics

Responsibilities

Home health visits
General patient assessments
Medication and disease education
Caregiver teaching
Monitoring current illness
Wound care
IV infusions

On-Call Requirements

Evenings and weekends
Average ~5 nights per month
- Typically 1 weekend (2 days)
- 3 additional weeknights
Examples of call duties:
- Hospital discharge visits
- IV teaching
- Wound care
- Phone triage/questions

Weekend Requirement

Weekend nurse covers most visits
Traveler typically on-call only
May see patients discharged unexpectedly or needing PRN visit
