Description

Position Overview: We are seeking a Hospice Nurse Practitioner with pediatric hospice experience for a part-time travel assignment based out of our Fresno location. Must have a DEA number. Key Responsibilities: Provide hospice home visits (4–5 visits per day) See patients in skilled nursing facilities (SNFs) and care facilities as needed Conduct face-to-face evaluations for Medicare recertifications in collaboration with the Medical Director Communicate with Case Managers regarding patients who require additional follow-up Round in the inpatient unit when needed Provide education to clinical team members as appropriate Schedule & Coverage Needs: Part-time position (1–2 days per week) Flexible scheduling — full or half days can be arranged Day shift only (Monday–Friday) No evenings or weekends available Additional details will be discussed during the Zoom interview process.
