Position: Physical Therapist
Contract Length: 13 Weeks
Hours per Week: 40
Shift: 5×8's
Position Summary
The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, and delivering high‑quality therapy services in accordance with physician orders and the established plan of care. The therapist will document all services according to professional and regulatory standards while ensuring excellent patient outcomes and adherence to all federal, state, and local requirements.
Key Responsibilities
- Perform comprehensive patient evaluations to determine functional status and therapy needs.
- Develop and implement effective treatment plans tailored to each patient’s goals.
- Provide physical therapy interventions in accordance with the established plan of care.
- Document assessments, progress notes, and treatment sessions accurately and timely.
- Communicate effectively with physicians, patients, families, and interdisciplinary team members.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain compliance with facility policies, professional standards, and regulatory requirements.
Minimum Requirements
- Current, active Physical Therapist licensure in the state of FL
- BLS
- One year of prior professional Physical Therapy experience
- Graduate of an accredited Physical Therapy program
