Job Overview

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing services in a clinic/ambulatory care setting. The RN will support patient care through assessments, triage, case management, and coordination with healthcare providers to ensure effective treatment and patient outcomes. This position requires working across two clinic locations connected to LCMC.

Key Responsibilities

• Provide professional nursing care in a clinical outpatient setting.
• Conduct walk-in and telephone triage for patient concerns and medical needs.
• Assess, plan, implement, evaluate, and document individualized patient care.
• Coordinate patient care in collaboration with patients, families, and healthcare teams.
• Manage case coordination based on PCP panels, including nurse visits and telephone follow-ups.
• Delegate patient care tasks to appropriate healthcare team members based on competency.
• Maintain accurate and timely clinical documentation within scope of practice.
• Support clinic operations and perform additional duties as assigned.

Requirements

• Minimum 2 years of nursing experience required.
• Strong clinical assessment, communication, and patient care coordination skills.
• Ability to work effectively in a fast-paced outpatient clinic environment.

Education

• Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN) from an accredited nursing program required.

Certifications & Licensure

• Active Registered Nurse (RN) license – State of New Mexico.
• Current BLS (Basic Life Support) certification required.
