Shift: 5x8-Hours (8:00 AM - 5:00 PM)

Additional Shifts: Options for four ten hour shifts a week

Job Description:

The Family Nurse Practitioner (FNP) will provide comprehensive primary care services to patients in a supportive and collaborative clinical environment. This role focuses on delivering high-quality, patient-centered care including preventive services, chronic disease management, and wellness visits. The FNP will work closely with a multidisciplinary team to promote health, improve patient outcomes, and support the mission of providing compassionate and community-focused healthcare. This is a full-time position with a weekday schedule, offering an excellent work-life balance with no weekend or holiday requirements.

Key Responsibilities:

• Provide comprehensive primary care services including patient assessment, diagnosis, and treatment planning.
• Conduct preventive care visits, wellness exams, and routine health screenings.
• Manage and treat acute and chronic health conditions.
• Develop and implement individualized care plans for patients.
• Educate patients and families on health maintenance, disease prevention, and treatment plans.
• Order, interpret, and review diagnostic tests and laboratory results.
• Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Follow clinical protocols, healthcare regulations, and best practices in patient care.

Required Qualification :

:- 1 year of Nurse Practitioner experience
