RN Case Manager | Anchorage, AK

Facility: Providence Alaska Medical Center
Location: Anchorage, Alaska
Duration: 13 Weeks
Schedule: Day Shift | 5×8s (Mon–Fri) | Shift between 07:00 – 17:00
Charting: Epic
Setting: Acute Care Hospital

 

Requirements:

  • Active Alaska RN License (required for auto offer)
  • BLS Certification
  • Minimum 1 year Acute Care experience
  • Epic EMR experience required

Responsibilities:

  • Discharge planning and care coordination
  • Utilization review and needs assessment
  • Ordering DME and developing patient care plans
  • Collaborating with interdisciplinary teams
