Job Summary
We are seeking a dedicated Occupational Therapist (OT) to provide rehabilitation services in a Skilled Nursing Facility (SNF) setting. The Occupational Therapist will evaluate and treat patients recovering from illness, injury, or surgery, focusing on restoring functional independence and improving the ability to perform activities of daily living.
Key Responsibilities
- Evaluate patients’ physical, cognitive, and functional abilities to develop individualized treatment plans.
- Provide therapeutic interventions to improve mobility, coordination, strength, and daily living skills.
- Implement rehabilitation programs that help patients regain independence and improve quality of life.
- Educate patients and caregivers on adaptive techniques, safety practices, and the use of assistive devices.
- Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals.
- Document patient assessments, treatments, and outcomes in accordance with facility policies and regulatory requirements.
- Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals to coordinate patient care.
- Participate in interdisciplinary care planning and discharge planning as appropriate.
Qualifications
- Active Occupational Therapist (OT) license in the state of practice.
- Minimum of one year of occupational therapy experience, preferably in a skilled nursing or rehabilitation setting.
- Graduation from an accredited Occupational Therapy program.
- Strong clinical assessment, communication, and patient care skills.
- Ability to work independently and as part of a multidisciplinary healthcare team.
