Shift: 5x8/DAYS (8A-5P: MON-FRI - will not work holidays or weekends unless approved prior to time/visit), Region: East (Baytown and surrounding areas), Milage: GSA Max, On-Call: None, GT: 40, Length: 8-13 weeks, Required Certs: BLS/First Aide, CANDIDATES MUST HAVE (2) YEARS RECENT LICENSED OT EXPERIENCE (EXCLUDES PRIVATE DUTY & NO NEW GRADS), THIS CAN BE THERE FIRST TRAVEL ASSIGNMENT AS LONG AS THEY HAVE HOME HEALTH OT EXPERIENCE, OT will need to call in for all missed visits and for frequency of eval or not able to contact patient, Scrubs: clean/professional with closed toe shoes - no facial jewelry except small nose stud and earrings, Charting: Wellsky, PLEASE HAVE ALL CANDIDATES LOOK INTO HOUSING BEFORE SUBMITTING.

- Point system for each visit: 1 for 1.

- Points required each week: 32-34.

- Paid drive time or GSA mileage: Paid for both.

- Is drive time considered in point system: Yes, drive time is considered when scheduling - if drive time pushes over 40 hrs, OT is paid.

- Will there be start of cares: No, if start of cares was to be added it would most likely be 3 pts due to AI.
